How to Complete Patient Portal Forms on your Cell Phone

  2. Sign in with your 麻豆原创 Portal credentials.

  3. Click the menu bar (grey bar with three blue lines).

  4. Click on “Forms”.

  5. Scroll down to “麻豆原创 Patient Forms” and complete the following forms: 麻豆原创 Medical History, 麻豆原创 Privacy Policy Acknowledgement, 麻豆原创 Release Authorization, and all other forms related to your visit (麻豆原创 Physical Form for Physicals, STI Test Consent Form for Sexual Health/UTI Visits, TB Questionnaire for TB Skin Tests, Women’s Health Form for Women’s Health Visits, etc.)

  6. Go to “Messages” for Class Excuses. Go to “Statements” for receipts from your visit.
